
Official Contest Rules
ELIGIBILITY: Open to all persons who are 13 years of age or older. Contest valid only in the United States. Board members and/or employees of Unlimited Creative Corporations (UCCP) and Please Don’t Die Black Men (PDDBM) are not eligible to enter or win a prize. All applicable federal, state, and local laws and regulations apply. Void where prohibited or restricted by law. NO PURCHASE NECESSARY. A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING.
CONTEST PERIOD: The Contest entry period begins at 8:00am on Friday, September 1, 2023 and ends at 11:59pm on Friday, November 3, 2023 (the "Contest Period"). Winners will be selected from entries properly submitted and timely received during the Contest Period.
HOW TO ENTER: During the Contest Period, schedule and fully complete an in-person, recorded interview with Ruby N Lewis, CEO of Unlimited Creative Corporations (UCCP). Limit One (1) Entry per person and/or business during the Contest Period regardless of whether a person uses more than one name, entity, or owns their own business. Each person and/or business must sign an on-camera release form.
VERIFICATION OF BIRTH NAMES AND BIRTHDATES: All On-Camera Release Forms must be submitted with a copy of either a driver's license, passport card, or other identification that shows the person's name and date of birth. All minors must have a parent/guardian sign an on-camera release form for them. Minors must also submit photo identification with their parent/guardian when submitting their on-camera release form. Names on the identification for all persons must match the names on the on-camera release forms. Stage, pen, and pseudonym names must be listed on the on-camera release forms in the correct space.
ALTERNATIVE MEANS OF ENTRY: You may also enter the Contest by mailing a signed UCCP On-Camera Release Form and a 60-second video to: Unlimited Creative Corporations (UCCP), 2700 Caples Avenue #3278, Vancouver, WA 98661. Alternate Means of Entry submissions must be postmarked during the Contest Period to be included within the random drawing, and no more than one alternate means of entry submission per person may be submitted during the Contest Period. No double submissions allowed. Either an in-person interview or an alternate means of entry may be submitted. All Alternate Means of Entry submissions must follow the Video Submission Guidelines listed below. Failure to submit proper forms or follow the Video Submission Guidelines below will result in disqualification of the submitted entry.
VIDEO SUBMISSION GUIDELINES: All entries must be submitted in digital format. Minors under the age of 18 must attach a signed parent/guardian participation consent form with their video submission or send it to info.uccp@gmail.com before submitting their video in order to be eligible for the contest. This form must also be resent with the submitted as per Alternate Means of Entry Rules listed above. All entries must either be submitted in English or, if in another language, include English subtitles. Videos must be unlocked and submitted in high resolution, 1920x1080, and mp4 format. Videos can be animation, doodling, live action, live stream, videographic, point and shoot, and/or screen recorded. Minors may work together in groups or submit entries as individuals. Any and all participants of a video must complete individually a UCCP On-Camera Release Forms, including editors, camera, and production crew. Failure to submit proper forms will result in disqualification of the submitted video.
The submitted video must answer the following statement: Describe a time when you suffered from circumstantial/situational depression. Other things to consider, but are not required, in your video may be: What did you do about it? Did you have support? How did you come out of it? What helped you through it?
The video should not be longer than 60 seconds. The 5-second title screen does not count towards this time limit. All entries should begin with a 5 second full-screen “title screen” that includes the following information: Lead Producer’s name, Voiceover name(s), Actors’/Actresses’ name(s), Camera and/or Production Crew name(s), Video Editor’s name(s), city, state, and Title of Video. Each video must have one Lead Producer to serve as the main point of contact. Recognition and prizes will be given to all producers listed on the entry form. No copyrighted materials (music, images, video clips, etc.) may be used for this contest unless you own the copyright or have a license to use the material for this contest. Written permission must be obtained and provided upon request for all copyrighted materials. The only compensation for submitting a video is the opportunity to be awarded a prize if the entry wins. UCCP will pay filmmakers no additional compensation nor be liable to the filmmaker under any circumstances. All persons, entities, and or businesses should keep at least one copy of their video. UCCP is not responsible for any films that are lost, damaged, or that do not upload properly. Entrants grant UCCP the right to use, reproduce, reprint, distribute, perform, and/or display the entrant’s project video without further compensation or notification to the entrant. UCCP maintains the right to reproduce, reprint, distribute, perform, display, or exhibit the project for advertising, publicity, and promotional purposes on their website, at conferences, or at any other venues. By accepting the prizes, winners agree to hold UCCP and all UCCP board members and employees harmless against any and all claims and liability arising out of use of such prizes. Winner assumes all liability for any injury or damage caused from participation in the contest or use/redemption of any prize.
WINNER SELECTION: Winners will be determined by random drawing to be held on or about Wednesday, November 8, 2023. Winners will be selected from entries received during the Contest Period. The potential winners will be notified via phone and email to the email address submitted with the potential winner’s entry within one (1) week after the drawing. Gift cards will be sent to the physical address located on the on-camera release forms. The drawing will be conducted by UCCP, the judge of the Contest, whose decisions on all matters relating to the Contest shall be final. In the event that a potential winner is disqualified for any reason, UCCP may, in its sole discretion, award the applicable prize to an alternate winner selected at random.
Prizes/Odds of Winning Prize: One (1) of Two (2) Visa Gift Cards worth $100. Approximate Retail Value (ARV) of each Prize: $100. Overall odds of winning depend on number of entries received during the Contest Period. No prize substitution or cash equivalent of prize. No prize transfer.
CONDITIONS: UCCP reserves the right, at its sole discretion, to cancel, terminate, modify or suspend the Contest (or portion thereof). UCCP also reserves the right at its sole discretion to disqualify the Entry of any individual. All federal, state and local taxes on prize and any expenses not specified herein are the sole responsibility of winner. Should a dispute arise regarding the identity of the claimant, UCCP reserves the sole right to determine the eligible party. By entering, entrant waives all rights to bring (and covenants not to bring) any claim against UCCP or any other(S) associated with this Contest. Entrants further agree to release and hold harmless UCCP from any and all liability arising from their participation in the Contest, or UCCP’S use of any entry information. Any disputes that may arise hereunder shall be governed in all respects by the laws of the State of Washington without regard to the conflicts of laws principles of any jurisdiction. Venue with respect to any such disputes shall be had in the state and federal courts of the State of Washington.
PUBLICITY AND RELEASE. As a condition of entry into the Contest, except where prohibited by law, each entrant and each winner by accepting a prize grants to UCCP, all right, title and interest in to publicize, broadcast, display and/or otherwise use the Contest entrant’s name, city, state, and biographical material (collectively, “Licensed Rights”) in any media for advertising and publicity purposes, without additional review, compensation, or approval of the winner. If Winner does not respond to notification, or is not present when prize delivery is attempted, such prize will be forfeited. By participating, entrants agree to be bound by the Official Rules and the decisions of the judges, which are final and binding in all respects.
OFFICIAL RULES/WINNERS LIST. For notification of the winners, send a self-addressed stamped envelope to: Unlimited Creative Corporations (UCCP), P.O. Box 3278, Vancouver, WA 98668. Requests for winners' lists must be received within one month after the end of the Contest Period.
